Treatment

In most cases, mild folliculitis heals spontaneously without any sign of scarring. Topical mupirocin can be used in these patients. Scarring or post inflammatory hyperpigmentation may occur if the infection deepens or if the patient excoriates the affected areas. Furunculosis can occur if the Staphylococcus infection spreads into the deeper part of the hair follicle.

Moderate to severe cases of folliculitis may be treated with systemic antibiotics in addition to topical antibiotics. For MSSA, cephalexin or dicloxacillin are recommended. In cases of MRSA doxycycline and TMP-SMX both work well. Pseudomonas responds well to ciprofloxacin. For patients who are symptomatic but culture-negative, benzoyl peroxide, topical clindamycin, and an oral tetracycline are recommended. Pruritus may be treated with a short term, low dose of hydrocortisone. A novel formulation that consists of plant extracts has been shown to exhibit potent anti-microbial activity against a broad spectrum of Gram positive and negative organisms, including MRSA.
